He already filled in the form and walk several times to the post office without any result.

I am so sorry Novia. Don't despair as it may still show up. Rachael, is there anyway a neighbor could have it? Did you ask them to poke around at the post office? I remember sending something to a friend in Philadelphia (about 35 minutes away) and the note for pick up was given to the neighbor as she wasn't home. It took her over a week to learn about the package. So strange things can happen.
